- Photo ID card issued by a government agency
Proof of Address:
- Aadhaar card
- Voter ID card
- Utility bills (electricity, telephone, water, gas)
- Bank statement
- Rent agreement
- Income tax assessment order
Proof of Date of Birth:
- Birth certificate
- School leaving certificate
- 10th standard marks card
- PAN card (if it includes date of birth)
- Driving license
Passport-sized photographs
Other Documents (as applicable):
- Marriage certificate (for name change)
- Divorce/separation certificate (for name change)
- Gazette notification (for name change)
- Parent's passport copy (for minors)
- Employer's certificate (for government/public sector employees)
- No Objection Certificate (NOC) (for government/public sector employees)
- Prior Intimation Letter (PIL) (for government/public sector employees)

* Important Notes:

Birth Certificate:
For applicants born on or after October 1, 2023, a birth certificate is the only accepted proof of date of birth.
ECR/ECNR:
ECR stands for Emigration Check Required, and ECNR stands for Emigration Check Not Required. The ECR status will be printed in the passport of applicants who fall in the ECR category.
Minor Applicants:
For minor applicants, address proof can be in the name of the parent(s), and the parent's passport copy should be submitted.
Government/Public Sector Employees:
Government/Public Sector/Statutory body employees need to submit either Identity Certificate in original, No Objection Certificate (NOC), or Prior Intimation Letter (PIL) as an additional document along with the application form.
Verification:
The officials at the Passport Seva Kendra (PSK) will verify the application form, photographs, and documents.
